Immigration Exam


Immigration Medical Examination

Dr. Erich Arias has been appointed as an authorized Civil Surgeon for the Department of Homeland Security by the U.S. Citizenship and Immigration service (USCIS). He is authorized to perform green card or immigration medical examinations (I-693 Form). This exam is very simple and in most instances requires two office visits.

The following is a list of items we suggest that you bring to your medical examination.


  • Valid Government-Issued Picture ID (Driver’s License or Passport).


  • USCIS I-693 Form. We can provide an I-693 Form if you don’t have one.
  • Please pre-complete the immigration physical form I-693 (Part 1 [Demographic portion] and Part 2 [about the interpreter if you have one]); Please print a “draft copy” on the page 1 and 2, complete your information as appears in your legal document (passport or ID); Family Name, Given Name, address, etc., and your immigration case number if you have one;  Your A-Number is the letter “A” followed by eight or nine numbers (such as A012345678).
  • You can find your A-Number and DOS Case ID on your immigrant data summary.
  • Bring this “draft copy” to your initial visit. You don’t need to sign this document on your initial visit.
  • Bring a proof of prior vaccination, if any. If you do not have any proof of vaccinations, do not worry as we will help you complete all your necessary immunizations.
  • TB testing and/or treatment records.
  • Prior mental health records.


The full USCIS exam is very simple and requires basic steps with 2 office visits.  You don’t need to pay for your follow up visit. The basic steps are as follows:

  1. A basic immigration physical exam and mental status evaluation is perform during the initial visit.
  2. An evaluation of your health status and immunization status will be reviewed by the civil surgeon.
  3. A blood test for syphilis and gonorrhea is required for all applicants of 15 years of age or older.
  4. Applicants under age 15 may not require syphilis  and gonorrhea testing but if for any reason at the discretion of the civil surgeon this test can be ordered if there is any suspicion of syphilis infection.
  5. All applicants age 2 and older are required to have a tuberculosis check. We will instruct you about this test the first day of your visit.
  6. Please note that in most instances the immigration exam process takes two visits; your initial visit as well a follow up visit to sign the USCIS form.
  7. Depending of the initial assessment and tests results further evaluation, treatment and or referrals will be needed.
  8. The cost of referrals, laboratory tests, X rays, treatment or immunizations other tests are not included in the price of the immigration exam.
